Why Wesley at Home
At Wesley at Home, we provide personalized health care so you are able to live comfortably and safely in your home during times of illness or decreased physical ability. Nurses, home health aids, home companions and therapists work with you to improve your overall quality of life.

Wesley at Home is a Medicare-certified home health agency serving Polk, Dallas, Warren, Boone and Story Counties. We are here to serve you 24 hours a day, seven days a week.

Wesley at Home accepts reimbursement from Medicare, Medicaid/Title 19, Elderly Waiver Services, long term care insurance and private pay. Certain requirements must be met prior to coverage by some of these sources. Wesley At Home nurses assist you in determining if you qualify free of charge. There is no additional cost for services on holidays, weekends or evenings.

Home Health Aides and Home Companions
Trained Home Health Aides at Wesley at Home provide hands-on, personalized care, including

The Home Companions at Wesley at Home provide assistance with domestic and personal needs. Services offered by companions create a pleasant home environment for you and your loved ones. These services include:

• Transportation to social events, medical appointments and shopping
• Other duties as assigned on a personalized care plan

Nursing and Therapy Services
Wesley at Home nurses provide specialized health services as prescribed by your physician, including

• Health assessments
• Medication management
• Management and prevention of illness
• Wound Care
• Other services prescribed by your physician

Wesley at Home also provides therapy services, including

• Physical Therapy
• Occupational Therapy
• Speech Therapy
